46 yr old male underwent 1 previous cosmetic primary rhinoplasty 6 years earlier. Soon after he noticed several deformities that worsened over time and he also developed nasal obstruction. Preop goals: Restore aesthetic proportions of the nose by adding projection back to the bridge while slightly decreasing projection of the tip, down rotate the up-turned nasal tip, increase and restore the length of the nose which had been overly shortened, decrease nostril show, improve the pinched bridge and tip, improve breathing. Procedure and outcome: Major open revision rhinoplasty using rib cartilage grafts, diced cartilage fascia grafts, composite cartilage and skin graft from the ear as part of a complete reconstruction to dramatically improve form and function.
