Primary Rhinoplasty Case #1773

This is a healthy, 44-year old male who presented with severe nasal deformity and obstruction. He experienced trauma to his nose as a child and had two subsequent septoplasties to improve the breathing and external appearance. Despite these interventions, his nose continued to curve. After visiting with me, he underwent an open, septorhinoplasty with rib cartilage reconstruction. His old septum was completely removed and the nose rebuilt with better function and aesthetics. At 8-months post-operative, he is very happy with the improved appearance and better breathing.

1. Notice the significant, leftward deviation of the nose - particularly, in the middle aspect. After, the tip is more narrow and the symmetry of the midvault has been restored.
2. The three-quarter view demonstrates saddling of the nose and the overly prominent and irregular tip cartilages. After, the bridge is smoothed and so is the tip.
3. Notice the saddling - or collapse - at the midpart of the nose from prior trauma. The after photo shows a smoothed profile with restored bridge. Additionally, the tip is counter-rotated to a more natural position.
4. The base view demonstrates a persistent tilt, though improved. The projection of the tip is better and the tip is smoothed. The scar at the columella is unnoticeable.
